Accessing an Isolation note
Please use the link below to the NHS 111 website for information and to get an isolation note. DO NOT come into the Practice for one of these thank you.
If you require a note for your employer as you have to isolate for 12 weeks or such time that the isolation ends, please download and complete the following document.

Online booking Suspended
Following advice form NHS England in regard to the Coronavirus outbreak the Practice has suspended the booking of appointments via the NHS App, please accept our apologises for any inconvenience this may cause and bear with our staff when ringing through as they will be handling an increased number of calls. We will review this against NHS England advice on a regular basis.
The Manor Surgery invites you to try the new NHS App online. You don’t need a smartphone or tablet to access it at www.bit.ly/nhsapptests
The NHS App is a simple and secure way to access a range of NHS services on a smartphone or tablet.
A lot of people don't own a smartphone or tablet, so the NHS App online enables you to use all the features of the NHS App on a website. This means you can use it on a laptop or desktop computer.
With the NHS App online you can:
- book and cancel appointments at your GP surgery
- order your repeat prescriptions
- view your GP medical record securely
- check your symptoms
- set your organ donation preferences, and more.
If you already use the NHSapp on your smartphone, this is also now up and running again, but you may need your linkage key, this was sent to you in the summer, if you have lost this or it does not work, pop into the practice between 9am and closing bringing photo ID with you and we can reprint it.
Try the new NHS App
If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet.
You can use the NHS App to check your symptoms and get instant advice, book appointments, order repeat prescriptions, view your GP medical record and more.
If you already use Online acces you can continue to use it until the end of November as we will be switching it off. You can use the NHS App along side the present system until then. You use the same account details we gave you when you first signed up for online access, if you don't have these please pop in and ask for a re-print or at the beginning of August we will be emailing them out.
